you must have the following information: |

of U.S. Citizenship
(Ex: Previous passport, Certified Birth Certificate, Original Certified Citizenship papers) all United States birth certificates submitted for minor and adult passport applications must contain the name of the parent(s) |

passport pictures ( 2 x 2 inches )
We do not take passport photos, try CVS, Walgreens, Photography Studio, or a Passport Photo Agency |

Fees: effective July 13, 2010
Department of State fees must be paid in either personal check or money order
Accepting Facility fees may be paid with cash, charge or check to the Village of Worth
The passport fees for adults eligible to apply by mail using form DS-82,
"Application for a U.S. Passport by Mail",
are $110 for the passport book and $30 for the passport card.
Average processing time for passports is 4-6 weeks.
$60.00 expedite fee
must be paid in either personal check or money order
and pre-paid two way overnight delivery envelope
optional--and available at the Village Hall
