WHEN APPLYING FOR A PASSPORT
you must have the following information:

1:
Proof of U.S. Citizenship
(Ex: Previous passport, Certified Birth Certificate, Original Certified Citizenship papers)
2:
Two passport pictures (2 x 2 inches)
(We do not take passport photos, try CVS, Walgreens, Photography Studio,
or a Passport Photo Agency)
3:
Proof of Identity
(Picture I.D. either your driver’s license or state I.D. Card)
4:
Your Social Security Number
Fees: effective July 13, 2010


Department of State fees must be paid in either personal check or money order
Accepting Facility fees may be paid with cash, charge or check to the Village of Worth

The passport fees for adults eligible to apply by mail using form DS-82,
"Application for a U.S. Passport by Mail",
are $110 for the passport book and $30 for the passport card.

Average processing time for passports is 4-6 weeks.

$60.00 expedite fee
must be paid in either personal check or money order
and pre-paid two way overnight delivery envelope
optional--and available at the Village Hall
