WHEN
APPLYING FOR A PASSPORT
you must have the following information:
1. Proof of U.S. Citizenship
(Ex: Previous passport, Certified Birth Certificate, Original Certified Citizenship papers)2. Two passport pictures (2 x 2 inches)
(We do not take passport photos, try CVS, Walgreens, Photography Studio, or a Passport Photo Agency)3. Proof of Identity
(Picture I.D. either your driver’s license or state I.D. Card)4. Your Social Security Number
FEES:
$100.00 FOR A TEN-YEAR PASSPORT (16 & Older)
($75.00
to U.S. Department of State--must be paid in either personal check or
money order,
$25.00 to Village of Worth--may be paid with cash, check or charge)
$85.00 FOR A FIVE-YEAR PASSPORT (Under 16)

$60.00 expedite fee (must be paid in either personal check or money order) and pre-paid two way overnight delivery envelope (optional--and available at the Village Hall)
Average processing time for passports is 10-12 weeks.
